Appointments & Payments
Appointments & Payments
All appointments are subject to availability.
Care at Functional Rehab is one-on-one, and your appointment time is reserved exclusively for you. When an appointment is missed, that time cannot be offered to another patient.
What to wear. Please arrive in comfortable, active clothing such as running or yoga gear. Treatment is hands-on and you'll need to be able to move freely.
Getting in. We're at 597 DeGraw St., Suite 2F, Brooklyn, NY 11217. Buzz 2F to be let into the building.
Payment. Payment is due at the time of service unless otherwise agreed. New patient appointments must be paid at the time of booking in order to hold the appointment time.
Card on file. Follow-up appointments require a valid credit or debit card on file at the time of booking. Your card is stored securely by our payment processor and is not kept in our office. By booking an appointment, you acknowledge and agree that this card may be charged for services rendered and for any late cancellation or missed appointment fee described below.
Cancellation, Rescheduling & Missed Appointments
Notice required. Please give at least 48 hours notice to cancel or reschedule an appointment.
Late cancellations and reschedules. Changes made inside the 48-hour window are subject to the full $185 visit fee.
Missed appointments. Not appearing for a scheduled appointment without contacting the office carries the same $185 fee.
Appointments already paid for. If you have already paid for an appointment and cancel inside the 48-hour window, that payment is not refunded.
Emergencies. Medical emergencies and other genuinely unforeseeable situations are handled case by case. Please contact the office as soon as you know you can't make it and we'll do our best to work with you.
Insurance & Superbills
Functional Rehab operates as an out-of-network provider and does not participate in-network with any insurance plan. Payment is collected directly from you at the time of service.
A superbill can be provided when appropriate so that you may seek reimbursement from your insurance company. Reimbursement depends entirely on your individual plan and how your insurer processes out-of-network benefits. We cannot guarantee that any portion of your care will be reimbursed.
You are financially responsible for all charges regardless of any insurance or benefit payments you may or may not receive.
Care & Outcomes
Chiropractic and rehabilitation care cannot be guaranteed to produce any specific result. You remain responsible for the cost of care regardless of outcome.
Outstanding Balances
Accounts more than thirty (30) days past due accrue interest at seven percent (7%) per annum. You are responsible for any legal or collection costs, including attorney fees, incurred in collecting a past due balance.
Intellectual Property
All website content, branding, text, graphics, and images are the property of Functional Rehab and may not be reproduced without permission.
Third-Party Links
Our website may link to third-party websites. We are not responsible for their content or policies.
Limitation of Liability
Functional Rehab shall not be liable for any damages arising from the use of this website.
Governing Law
These terms are governed by the laws of the State of New York.
