We offer two ways of paying at Functional Rehab
Self Pay
New Patient Sessions (60 minutes) $225
Existing patients (Chiro and PT) $160
Existing patients (Adjustment only) $85
*HSA/FSA accounts may be used for payment
We also accept Venmo and Zelle

Out-of-Network Insurance
We accept many major plans with out-of-network benefits. But we are not in-network with any plans.
If you have insurance, please send a photo of the front and back of the card along with your date of birth to Jason@functionalrehabny.com. Our billing department will be able to verify your benefits and tell you exactly what your responsibility will be per session.
*We do not accept Medicare/Medicaid, no-fault or workers comp
